Myocardial Infraction

Introduction

Myocardial infarction (MI), commonly known as a heart attack, is a life-threatening condition where blood flow to a part of the heart is blocked for long enough to cause damage or death of heart muscle.

Definition 

Myocardial infarction occurs when one or more of the coronary arteries becomes blocked, leading to insufficient blood flow to the heart muscle. This causes ischemia (lack of oxygen) and necrosis (death) of myocardial tissue, resulting in severe chest pain and other symptoms.

Pathophysiology of Myocardial Infarction

The pathophysiology of myocardial infarction involves several key steps:-
  1. Atherosclerosis:- Over time, fatty deposits (plaque) build up in the coronary arteries, narrowing them and reducing blood flow to the heart.
  2. Plaque Rupture:- A sudden rupture of the plaque can lead to the formation of a blood clot (thrombus) that completely blocks the artery.
  3. Ischemia:- The blocked artery prevents oxygen-rich blood from reaching the heart muscle, causing ischemia.
  4. Infarction:- Prolonged ischemia results in the death of heart muscle cells (necrosis), leading to myocardial infarction.

Types of Myocardial Infarction

STEMI (ST-Elevation Myocardial Infarction)

Definition:- A severe type of heart attack where there is a complete blockage of a coronary artery, leading to significant damage to the heart muscle.
Symptoms:- Severe chest pain, shortness of breath, sweating, nausea, and lightheadedness. ST-segment elevation is seen on an ECG.

NSTEMI (Non-ST-Elevation Myocardial Infarction)

Definition:- A less severe type of heart attack where there is a partial blockage of a coronary artery, causing less damage to the heart muscle.
Symptoms:- Chest pain, shortness of breath, fatigue, and dizziness. There is no ST-segment elevation on an ECG, but other markers of heart damage, like troponin, are elevated.

Silent Myocardial Infarction

Definition:- A heart attack that occurs without the typical symptoms, often going unnoticed.
Symptoms:- May include mild discomfort, fatigue, or no symptoms at all. Often diagnosed later through ECG or imaging studies.

Causes  of Myocardial Infarction

  1. Atherosclerosis:- The primary cause of myocardial infarction is the buildup of plaque in the coronary arteries, reducing blood flow.
  2. Thrombosis:- The formation of a blood clot due to plaque rupture can completely block the coronary artery.
  3. Coronary Artery Spasm:- Sudden tightening of the coronary artery can reduce or stop blood flow to the heart.
  4. Cocaine Use:- Can cause coronary artery spasms, leading to a heart attack.
  5. Other Factors:- Hypertension, high cholesterol, diabetes, smoking, obesity, and a sedentary lifestyle increase the risk of atherosclerosis and subsequent myocardial infarction.

Symptoms

  1. Chest Pain (Angina):- Often described as pressure, tightness, or squeezing in the chest, resulting from ischemia.
  2. Radiating Pain:- Pain may radiate to the left arm, neck, jaw, or back due to shared nerve pathways (referred pain).
  3. Shortness of Breath:- The heart’s reduced pumping efficiency leads to pulmonary congestion and dyspnea.
  4. Sweating (Diaphoresis):- The body’s sympathetic nervous system responds to stress.
  5. Nausea and Vomiting:- Stimulation of the vagus nerve due to the heart’s distress.
  6. Fatigue:- Reduced cardiac output and poor tissue perfusion result in exhaustion.
  7. Dizziness or Lightheadedness:- Low blood pressure and poor cerebral perfusion cause these symptoms.
  8. Silent Symptoms:- Especially in diabetics and older adults, symptoms may be atypical or mild, including fatigue or mild discomfort.

Diagnostic Methods 

Clinical Evaluation
  1. History and Physical Examination:- Assessment of symptoms, medical history, and risk factors. Physical examination may reveal signs such as abnormal heart sounds or lung congestion.
Electrocardiogram (ECG)
  1. ECG Changes
    • ST-Segment Elevation:- Indicates STEMI.
    • ST-Segment Depression or T-Wave Inversion:- May indicate NSTEMI.
    • Pathological Q Waves:- Suggest previous infarction.
Blood Tests
  1. Cardiac Biomarkers
    • Troponin I and T:- Highly specific markers for myocardial damage. Normal range: 0-0.04 ng/mL.
    • Creatine Kinase-MB (CK-MB):- Elevated in myocardial infarction. Normal range: 0-3 ng/mL.
    • Myoglobin:- Early marker of muscle injury. Normal range: 0-85 ng/mL.
Imaging Studies
  1. Echocardiography
    • Purpose:- Assesses heart function, wall motion abnormalities, and ejection fraction.
    • Findings:- Reduced movement of the heart wall in areas affected by the infarction.
  2. Coronary Angiography
    • Purpose:- Visualizes the coronary arteries to identify blockages.
    • Findings:- Shows location and severity of arterial blockages.
Stress Testing
  1. Exercise or Pharmacological Stress Test
    • Purpose:- Assesses the heart’s response to stress and identifies areas of ischemia.
    • Findings:- ECG changes or symptoms induced by stress may indicate coronary artery disease.

Management of Myocardial Infarction

Non-Pharmacological Management

  1. Positioning:- Keep the patient in a comfortable position, usually semi-Fowler’s, to reduce the workload on the heart and improve breathing.
  2. Oxygen Therapy:- Administer supplemental oxygen to maintain adequate oxygenation and reduce myocardial oxygen demand.
  3. Lifestyle Modifications:- Encourage a heart-healthy diet, regular physical activity, smoking cessation, and weight management.

Pharmacological Management

  1. Antiplatelet Agents
    • Aspirin:- Inhibits platelet aggregation, reducing the risk of clot formation.
    • Clopidogrel (Plavix):- Used in combination with aspirin for dual antiplatelet therapy.
  2. Anticoagulants
    • Heparin:- Prevents further clot formation during acute management.
    • Enoxaparin (Lovenox):- A low molecular weight heparin used to prevent clot progression.
  3. Thrombolytics
    • Alteplase (tPA):- Used in STEMI to dissolve the existing clot and restore blood flow.
  4. Beta-Blockers
    • Metoprolol (Lopressor):- Reduces heart rate and myocardial oxygen demand.
  5. ACE Inhibitors
    • Lisinopril (Prinivil):- Lowers blood pressure and reduces heart strain.
  6. Statins
    • Atorvastatin (Lipitor):- Lowers cholesterol levels and stabilizes plaques.
  7. Nitrates
    • Nitroglycerin:- Relieves chest pain by dilating coronary arteries.

Surgical Management

  1. Percutaneous Coronary Intervention (PCI)
    • Angioplasty:- A balloon is used to open the blocked artery.
    • Stent Placement:- A stent is placed to keep the artery open.
  2. Coronary Artery Bypass Grafting (CABG)
    • Surgical Bypass:- Grafting vessels from other parts of the body to bypass blocked coronary arteries.

Assessment

    • Blood Pressure:- Frequent measurements to detect hypotension or hypertension.
    • Respiratory Rate:- Observe for increased rate indicating respiratory distress.
    • Temperature:- Monitor for fever which may indicate infection.
  2. Pain Assessment
    • Location, Intensity, Duration:- Use pain scales to assess chest pain and its characteristics.
  3. Cardiac Monitoring
    • ECG:- Continuous monitoring for arrhythmias and changes indicative of ischemia.
    • Heart Sounds:- Auscultate for murmurs, gallops, or pericardial friction rub.
  4. Respiratory Assessment
    • Breath Sounds:- Auscultate for crackles indicating pulmonary congestion.
    • Oxygen Saturation:- Continuous pulse oximetry to ensure adequate oxygenation.
  5. Peripheral Circulation
    • Pulse Quality:- Assess for strength and regularity.
    • Capillary Refill:- Evaluate for peripheral perfusion.
Interventions
